Online Patient Registration

Physicians Quality Care now has two ways to decrease your wait time and make your visit more pleasurable.  You may come in, register, then leave.  Our patient concierge will take your cell number and text you when it’s your turn to be seen.  Or you can click the BOOK NOW button to the right and reserve your place in line from home.  We’ll email you a confirmation - even send you a text message before your appointment time.  When you arrive, you’ll be placed near the top of the waiting list.  You may also print out the necessary forms by clicking on Patient Registration and Financial Policy and fill them out before you come, or you may handle the paperwork after you arrive.  Upon arrival, you will be asked to present your driver’s license and insurance card, and to confirm your birth date.  Then your registration process will be complete. You may also schedule appointments in advance to see Donna Wallis for Women’s Health concerns or for your annual exam.  And for other specialized appointments.  Just click BOOK NOW.
